Affidavits of Paternity:

A paternity affidavit may be completed at the hospital within 72 hours of a child’s birth or at the local health department any time before a child is emancipated. If paternity is established by paternity affidavit, the Department of Health will add the father’s name to the child’s birth certificate.

Parents who wish to sign a paternity affidavit must make an appointment with Vital Records. Both parents must be present and have valid and acceptable documentation. Call 260-925-2220 for information or to make an appointment.

 

Acceptable Identification required for completion of a Paternity Affidavit

 

You must present 1 of the following as proof of identity:

  • Valid State Driver’s License
  • Valid State Identification Card
  • Valid Passport

Copy of new birth certificate -  $10

Paternity Affidavit Upon Marriage (filed upon the marriage of the parents of a child born out of wedlock) filing fee - $10
